Quality
QA scorecard weighting
Build the scorecard, set the weights, mark the auto-fails. The tool refuses to produce a score while the weights do not total 100 — which is where most quality programmes quietly go wrong.
Score = Σ(weight × criterion) ÷ Σweight · any auto-fail ⇒ 0
Criteria
Weights total 100%

An auto-fail should be reserved for things that harm the customer or breach an obligation. If more than two criteria carry one, the scorecard is being used to enforce preference rather than standards.
